Other drug interactions to consider: (1) Diabetes medications — possible additive glucose-lowering. (2) Anticoagulants — case reports of garcinia affecting bleeding. (3) Statins — garcinia may interact with statin metabolism in some cases. (4) Pre-existing liver disease — avoid entirely. Given the weak efficacy and documented hepatotoxicity case reports, the risk-benefit for adding garcinia to any prescription regimen is unfavorable. Coordinate with prescriber and consider that the modest possible benefit doesn't justify the interaction risks.

The most-cited rigorous RCT of garcinia cambogia for weight loss. A 12-week double-blind trial of HCA (1500 mg/day) vs placebo in 135 overweight subjects. Found NO statistically significant difference in weight loss or body fat percentage between groups. This is the foundational negative evidence base. Published in JAMA. Despite this null finding, garcinia continued to be marketed aggressively as a weight-loss supplement.

A systematic review and meta-analysis of 12 RCTs of garcinia/HCA for weight loss. Found a small statistically significant weight loss (~0.88 kg) in the garcinia group vs placebo. Authors specifically noted: trial quality was poor, magnitude of effect was small, clinical relevance was uncertain. The honest framing reference — small effect of questionable clinical significance with poor underlying trial quality.
