How much bacteriostatic water for semaglutide?

FAQ

There is no correct volume, because the water is a solvent rather than an ingredient. It sets the concentration, and the concentration sets how many syringe units a given milligram figure measures out to. The amount of semaglutide drawn for any dose is identical whichever volume is used — only the number on the barrel changes.

Why any volume works

The powder in the vial is a fixed mass, and reconstituting it changes where that mass sits rather than how much of it there is. A larger volume spreads the same milligrams across more liquid, so each milligram occupies more barrel — the figure on the syringe goes up while the quantity drawn stays put. What the choice really trades is readability: a 100-unit insulin syringe holds 1 mL, so a pairing that pushes a single measurement past 100 units turns one draw into two.

Reference table: volume, concentration, and syringe units

Units on a 100-unit (1 mL) insulin syringe. The dose points are the labelled Wegovy escalation steps, listed because they are the figures the approved schedule uses — not as a recommendation to use any of them. 5 mg and 10 mg are the two vial sizes most commonly listed.
Vial + waterConcentration0.25 mg0.5 mg1 mg1.7 mg2.4 mg
5 mg + 1 mL5 mg/mL5 units10 units20 units34 units48 units

10 mg + 1 mL10 mg/mL2.5 units5 units10 units17 units24 units
10 mg + 2 mL5 mg/mL5 units10 units20 units34 units48 units

The arithmetic is: units = (dose in mg ÷ concentration in mg/mL) × 100. Note the 10 mg + 1 mL row — at 10 mg/mL the smallest labelled step measures 2.5 units, which is a hard measurement to read accurately on a 100-unit barrel. That is the legibility trade-off the volume choice is actually about.

To run these figures for a vial size or volume not tabulated here, use the dosage calculator.

Bacteriostatic versus sterile water

The two waters differ by one ingredient: bacteriostatic water carries 0.9% benzyl alcohol as a preservative, and sterile water carries nothing. That single difference decides how many times a vial can be opened, because the preservative is what suppresses whatever a needle carries in. With semaglutide the arithmetic makes this concrete rather than theoretical — at the labelled ladder, a 10 mg vial is a multi-month supply and gets entered a couple of dozen times before it is finished, which is a very different exposure from a compound used up inside a week.
