How do semaglutide pills compare with the injection?

FAQ

Labels estimate that 0.4–1% of a Rybelsus dose and 1–2% of an Ozempic or Wegovy tablet dose reaches the bloodstream, against 89% for injected semaglutide. Three tablet products are FDA-approved, at far larger milligram doses. Per the OASIS 4 paper, oral semaglutide 25 mg produced a 13.6% mean weight loss at week 64, against 2.2% on placebo.

Figures as read on DailyMed on October 7, 2026. The Rybelsus and Ozempic tablets label states that the two tablets are not substitutable on a mg-to-mg basis, and no tablet milligram figure converts to an injection one.

For the adverse events reported at these label doses, see semaglutide side effects.

How a semaglutide tablet is absorbed

Each tablet is co-formulated with SNAC (salcaprozate sodium), which the labels name as an absorption enhancer. The Rybelsus and Ozempic tablets label states that absorption "predominantly occurs in the stomach". A 2018 study in Science Translational Medicine located it at the stomach lining next to the tablet, and found that SNAC protects semaglutide from enzymes by local buffering and only transiently raises absorption.

That mechanism is why the tablets come with conditions. The labels direct the tablet to be swallowed whole on an empty stomach in the morning, with no more than 4 ounces of water, and at least 30 minutes before food, other drinks or other oral medicines. In a study the label reports, absorption was higher when the tablet was taken with 50 mL of water than with 240 mL.

Why the milligram numbers are so different

Because so little of a tablet is absorbed, its dose is many times the injected one. The Wegovy label sets 25 mg once daily for the tablet, 175 mg a week, against 2.4 mg once weekly for the injection, about 73 times the milligrams. Rybelsus at 7 mg daily is 49 mg a week, about 98 times a 0.5 mg weekly Ozempic injection. Both ratios are Prof. Peptide's arithmetic from the label doses; the labels themselves present no conversion between the forms.

What the tablet trials found

PIONEER 1 (Diabetes Care, 2019) randomized 703 adults with type 2 diabetes to oral semaglutide 3, 7 or 14 mg or placebo for 26 weeks. Against placebo, HbA1c fell by 0.6, 0.9 and 1.1 percentage points at the three doses (the trial's treatment-policy estimate).

OASIS 4 (New England Journal of Medicine, 2025) randomized 307 adults with overweight or obesity, 205 to oral semaglutide 25 mg and 102 to placebo. Per the paper, mean body weight changed by −13.6% and −2.2% at week 64, an estimated difference of 11.4 percentage points. The Wegovy prescribing label reports the same trial as Study 7 with −13.6% and −2.4%, a difference of 11.2 points, from its own model with missing data imputed.

Which other GLP-1 drugs come as pills

Foundayo (orforglipron) is an oral GLP-1 receptor agonist that the FDA approved on April 1, 2026 (NDA 220934). It is not a peptide: its label gives a molecular weight of 902 g/mol and a ring-based chemical name with no amino-acid chain. The label reports 77% absolute bioavailability after a 0.8 mg dose, and the tablet is taken with or without food.

No approved oral tirzepatide exists. The only oral tirzepatide study found for this page is a 2026 tablet-formulation paper that measured exposure in beagle dogs, and no oral study of retatrutide was found. Compounded sublingual semaglutide does exist: a 2026 survey in Annals of Pharmacotherapy found 17 single-ingredient compounded semaglutide or tirzepatide products that differ from the approved ones, 82% of them sublingual, and described their efficacy and safety as largely unknown.

What the label figures do not cover

Every figure above belongs to an approved product with SNAC in a defined tablet. Research-market semaglutide sold as powder, capsules or drops is none of these products, and nothing in the labels describes how much of it would be absorbed.
